A man has been stabbed in the back in what police believe is a random attack inside a waiting room at a busy inner Sydney hospital.

The 45-year-old was in the triage area of Royal Prince Alfred Hospital at Camperdown late on Tuesday night when he was stabbed by a woman.

The pair aren't believed to be known to each other - the man had been brought in from a hospital in Wagga Wagga for unrelated treatment, while the woman had arrived by ambulance from a home in Concord.

The victim underwent emergency surgery and is in a stable condition.

The 29-year-old woman was arrested and is under police guard in hospital, where she will undergo a mental health assessment.

"The incident is not being treated as a targeted or provoked attack," police said in a statement on Wednesday.
